Clinical Navigator

This position is responsible for delivering clinical behavioral health assessments and evaluations within the community to determine the necessary levels of service for potential patients, ensuring they are referred to the most suitable service or resource that meets their needs.  

Builds a network of support for the hospital and enhances its capacity to address the needs of the public, including various stakeholders such as physicians and their office staff; community members, schools, religious organizations, civic groups, local employers, legislators, other healthcare and human service providers, and the media. Additionally, seeks collaborative opportunities to enhance the quality of life for residents in the hospital’s service area.

 

KEY RESPONSIBILITIES:

  • Communicates and distributes information regarding the Hospital’s services to community healthcare organizations in a positive educational manner.   
  • Keeps updated on referral sources and engages effectively with patients, colleagues, and other staff regarding appropriate patient treatment options. 
  • Understands and communicates the disease pathology related to behavioral conditions and their treatment models.
  • Facilitates the patient's access to the appropriate level of care and setting.
  • Develops and maintains effective working relationships with physicians, nursing staff and health professional affiliates in designated community areas.
  • Demonstrates strong abilities in assessing client's crisis and clinical indicators.
  • Compiles a written report of assessment findings promptly (immediately after completing the assessment) and includes recommendations for the necessary level of care and treatment needs.
  • Implements appropriate interventions during assessments and crisis situations involving clients, family/friends, and other professional staff.
  • Evaluates the necessity for involuntary hospitalization assessments and contacts the county crisis team, law enforcement or 5150 certified personnel as required to ensure the safety of the client and others.
  • Provides relevant clinical information to the physician and Intake staff who will be assessing the patient, ensuring a seamless transition for the patient.
  • Remains knowledgeable about legal regulations concerning EMTALA/COBRA and abuse/neglect reporting.
  • Implements marketing strategies and initiatives to promote the profit growth and expansion of the organizations’ products/services.
  • Maintains relationships and gathers competitive information on products and services in order to maintain and develop market share. 
  • Executes communication strategies, plans, and materials consistent with the organization’s marketing image.
  • Represents the organization at major public events.
  • Schedule is Monday - Friday, from 8:30am - 5:00pm

Knowledge and Experience:

  • Valid Nevada State LPN or RN license required.
  • Bachelor’s Degree in Marketing, English, Journalism, or other related discipline preferred.
  • Minimum one (1) year experience in marketing, community development, public relations or crisis communications required.
  • Valid Nevada Driver’s License and car insurance required

Skills and Abilities:

  • Effective oral and written communications.
  • Knowledge of local, state, and federal political processes.
  • Knowledge of patient rights and laws pertaining to mental health.
  • Knowledge of printing procedures, graphic arts, photography, and desktop publishing.
  • Proficiency with software and/or equipment. (Microsoft Office applications including Outlook, Word, Excel, and PowerPoint)
  • Strong organizational skills with ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines.
